Medical Examination for Admission

Here is a series of steps that will guide you through the medical examination that is required for admission to the university.

 

Medical and Dental Services

Avail of the health services at a discounted rate. 

 

 

Requirements for New Students (including transferees)

  • Permit for Medical Examination issued by Admissions Office

  • Pay the total fee of Php 335.00 at VSU Hospital’s Cashier

Process

Step 1  - @ Information Counter
-Present permit for medical exam obtained from the University Student Services Office
-Obtain Laboratory Request
-Fill-out Medical Exam Form and wait for your priority number

Step 2 - @ Cashier Window 3
(Note: You may undergo the same laboratory exams in clinics outside VSU Hospital)
-Pay for the following laboratory fees:

CBC: PhP 95.00
Urinalysis: PhP 35.00
Stool Examination: PhP 35.00
Chest X-Ray: PhP 170.00
TOTAL: PhP 335.00

Step 3 - @ Laboratory Rooms
Skip this process if laboratory exams from other clinics have already been done. 

Step 4 - @ Dental Clinic
Skip this process if laboratory exams from other clinics have already been done.

Step 5 - @ Nurses Station – OPD
-Height and weight measurements
-Vital signs
-Eye examination

Step 6 - @ Physician's office
-Bring laboratory and Chest X-Ray results to the physician for examination
- Wait for release of medical or physical exam certificate

Step 2 -  @ Window 1 – Medical Records Personnel
-Retrieve old chart (Medical records personnel forwards record to Dental Clinic)

Step 3 - @ Dental Clinic
-Annual Dental Exam

Step 4-  @ OPD Nurses Station
-Height and weight measurements
-Vital signs
-Eye examination

Step 5 - @ Physician's Office
-Submit complete record of laboratory and Chest X-Ray results
-Physical Exam
-Release of Physical Exam Certificate
